You have never been denied a U.S. visa.
and
You have never been deported from or denied entry into the U.S.
and
You are going for 90 days or less for business or pleasure.
You DO NEED a visa if:
You have a handwritten passport.
Or
You are not a citizen of a Visa Waiver country or Canada.
