a ust tried giving you a call but missed you - and I are working on getting all our approvals for a trip to
starting February 3, and on the foreign travel questionnaire it refers to a requirement to complete a "DO1-504" if
you have TS/SCI clearance, which we both do. I can't find that form anywhere online — is that something we actually need
to do, and if so, could you please send us a form or example, or point us in the right direction?
