List of Discrepancies to
repair on Serial #472
I. #1 Engine MO4 - PT Blades, P/N 2 292 81 043 0, S/N 49 per logcard are original to this
module — PT Blades RETIRE in 318.1 hours. It Is an absolute condition to closing that
Turbomecca provides a letter referencing extension or Service Bulletin manual update
increasing Service Level Life, to provide additional 300 or 600 hours on PT Blades.
2. Need positive Soap oil sample results for engines in writing. (metallurgy results).
3. Update W&B — Chart "C" has one boarding step removed, but both are installed. Entry
for 11 APR 08 in pencil — erase — and use entry for 22 DEC 06: Weight: 8 271.25 lbs•
C.C.: 207.19; Arm: 17 137 30. (Pencil log entry is not exceptable, Aetna needs to reissue
updated Weight and Balance using ink pen.)
4. L/H & R/H Main Landing Gear static wick straps missing. (install new)
5. Standby Compass and Right & Left Compass Cards not legible, the ink is very faded (Sun
Bleached), ( Aetna needs to research compass swing data and provide new card.)
6. Required placards missing from L/H & R/H Main Landing Gear wheel assemblies. (install
new placards)
7. HELOTRAC updates required by Aetna on following items #7 - #20.
8. Engine Terminal Stud Inspection (243107A). (Aetna needs to update
HELOTRAC if this is competed)
9. Check interior cockpit & cabin emergency lights — past due (330001). (Aetna needs to
update HELOTRAC if this has been completed).
10. Inspect MGB support beams — (532014). (Aetna needs to update HELOTRAC if
this has been completed)
I. Inspect Low Rotor Speed Warning System — (656201). (Aetna needs to update
HELOTRAC if this has been completed).
12. Update HELOTRAC to reflect that the High Pressure Turbine Blade were replaced with
new during the repair of the #1 Engine MO3 module 28 SEP 09. Module had TSN:
5,460.0, TSO: 2,669.2, CSN 6,945.0 and CSO: 3,271.0. (Aetna needs to update
HELOTRAC if this has been completed)
13. Update HELOTRAC — need to update L/H Main Landing Gear Actuator Piston Rod End
P/N 1945-210, S/N HSM1646 installed ® 14,658 Landings (15 MAR 2010)
14. Update HELOTRAC — need to update Black M/R Blade S/N: A086-02328, has 152.7

hours less then indicated. (Aetna needs to update HELOTRAC if this is correct).
15. Update HELOTRAC — need to add information for all bifilar weights.
16. Update HELOTRAC — T/R blade paddle and horn should be A334-03216A not A334-
03126.
17. T/R D/S Bearing Support Assembly Logcard (P/N 76361-04007-042, S/N SD-0118) does
not indicate 5 year overhaul or bearing change complied with at A/C TT: 6,796.6 (2 JUL
10). New bearing S/N: 13626-66894.
18. Need to correct Time Remaining in HELOTRAC for #1 Engine MO5, S/N 01477TEC - •
should be 2,928.3 hours (A/C TT: 6893.5 hours).
19. Need to add S/N to HEOTRAC for #2 Engine HELOTRAC Code 7400028 — engine S/N
20533.
20. Need to add M/R Blade S/Ns to HELOTRAC.
21. Did not see logcard for DC Starter Generator P/N 23081-043, S/N 2837. Aetna needs to
generate a card for this item.
22. Corrosion spot under paint vicinity L/H baggage door frame at sta 215 seam (forward
upper corner). Clean and dress area of concern.
23. Two corrosion spots L/H air inlet door DZUS fuselage socket arca and L/H engine cowl
DZUS fuselage socket. Clean and dress arca of concern.
24. Light corrosion two spots: R/H air inlet door DZUS fuselage socket and forward engine
cowl DZUS fuselage socket. Clean and dress area of concern.
25. Light Corrosion on R/H aft tailboom static wick base. Clean and dress area of concern.
26. Inner tailcone, light surface corrosion areas vicinity of access panel lower nut plates &
small stiffening longeron — bottom inboard of access panel between sta 316 — sta 333. Clean
and dress area of concern.
27. R/H Main Landing Gear:
a. Tire worn — small cut mid tread. (replace tire)
b. Wheel missing required placards. (install new placards)
c. Pro-seal / RTV wheel nut cotter pin. (pro seal area)
d. Spherical bearing has rust coming out. (inspect Bearing and replace if out of limits)
e. Adel clamp cluster "brake hose/landing light harness" — rubber deteriorating —(
replace clamp).
28. Replace windshield wiper blades, pro-seal posts.
29. L/H Main Landing Gear:
a. Wheel missing required placards.
b. Main wheel brake wear key missing.
c. Pro-seal wheel nut cotter pin.
30. R/H Main Landing Gear:
a. Tire worn — small cut mid tread. ( if cut is beyond manufactures specs, replace tire)
b. Wheel missing required placards. (install placards)
c. Pro-seal / RTV wheel nut cotter pin. (pro seal area)
d. Spherical bearing has rust coming out. (inspect Bearing, and replace if beyond
limits according to Maintenance Manual)
e. Adel clamp cluster "brake hose/landing light harness" — rubber deteriorating —
(replace add clamp).
31. #1 Engine Area:
a. Needs compressor wash — dirty.
b. Various "red" rubber add clamps rubber deteriorating — (replace clamps).

32. L/H & R/H engine cowl forward corners — interior stainless steel fire shield rolled up
—(Ruff unsafe edges need to by filed down).
33. #2 Engine Area:
a. Remove & Replace both adel clamps on fire extinguisher lower tube.
b. Remove & Replace adel clamp on forward R/H engine mount.
34. #1 & #2 Exhaust ejectors worn through. (Repair in accordance with Maintenance Manual)
35. Rotor Brake Disc out of limit — pads slightly less than 50% remain. (measure rotor disc, if
out of limits according to Maintenance manual, replace Disc as required)
36. Investigate noise during engine spool up from 70-100% (suspect AC Generator splines
may need lubrication ).
37. Suspect Nose Landing Gear seeking until it finally goes into the up-lock position.
38. Nose landing gear actuator seeping/leaking. (reseal Nose landing gear actuator, this should
remedy item #37 above)
39. All Main Rotor Blades — spot paint where exposed kevelar exists, to include exposed
leading edge abrasion strip Main Rotor Blades skin scams. (clean and dress in accordance
with Maintenance Manual
40. #2 Engine T5 gauge does not test — Digits do not change — troubleshoot (replace or over
haul T5 gauge)
41. The following Items Due July 31, 2011 need to be completed by Aetna,:
42. 30 Day Check of portable fire extinguishers due 30 JUL 11 (262001).
43. Lube AC Generator Splines at 7,396.6 hours or 31 • (240020).
44. Battery Overtemp Thermal Switch due [1 July 20 243103).
45. Op-test Essential BUS Recove S stem due (243104)
46. Inspect First Aid Kit due (256003 .
47. Inspect 1g & 2od Stage Hydraulic Systems due (290005).
48. Check Landing Gear Emergency Blowdown Bottle Cable due (323101).
49. Inspect cockpit & cabin doors due (521001).
50. 12 Month inspection of STC SR01113NY-D door pin kit due (521009).
51. Inspect AFT engine cowl, latch hook & latch fittings due (532007).
52. Low T/R Blade balance check due 31 July 2011 (652114).
53. Lube Swashplate due 31 July 2011 (65 1420)
54. Change MGB oil due 31 July 2011 (661021).
55. Op-test gearbox chip detector warning s stem due (666404).
56. Inspect all control rods, due 31 July 20 (670003).
57. Inspect Flight Controls, due 33 July 20 (670007
58. Inspect Collective Control Stick for cracks, due (671201).
59. Inspect M/R Servos & supports, due 31 (671515A).
60. T/R Control Pedal & Support Arm Control Rod inspection/Lube, due 31 July 2011
(672007).
61. Change #1 & #2 Engine Oil, due (790005A & 790005B).
62. Lube/Inspect #1 & #2 Engine Oil Cooler internal Splines, due 31 July 2011(791012A &
