Consent Form Instructions for
International Travel by Minors
S Options
Passengers under the age of 18 traveling to or from the United States, when not accompanied on the trip by
both parents or legal guardians, must bring with them a signed, notarized Consent Form for International
Travel by Minors rConsent Form") and/or any other required documentation described below. Flight Options
also requires a copy (via facsimile or other method of delivery) of the Consent Form and any additional
required documentation at the time the child's reservation is made, or shortly thereafter. The minor is
required to bring the original signed, notarized copy of the Consent Form and other required documentation to
the airport and have it available with his/her raised seal birth certificate and passport or photo identification.
Copies of birth certificates will not be accepted. All documents must be certified copies or raised seal
originals. The information provided to Flight Options at the time the minor's reservation is made will be
compared with the original signed, notarized Consent Form and other required documentation. The minor will
retain the original Consent Form and other required documentation for the purpose of entering the foreign
country.*
1. Minor traveling with only one parent: Passengers under the age of 18 traveling between the United States and
any foreign country, when accompanied on the trip by only one parent must provide Flight Options with a copy of
the Consent Form signed by the parent who is not traveling and notarized by a Notary Public.
2. Minor traveling alone: Passengers under the age of 18 traveling between the United States and any foreign
country when not accompanied on the trip by any parent must provide Flight Options with a copy of the notarized
Consent Form signed by both of the parents. [Note: Minors who are under 14 years of age are not permitted to
travel without being accompanied on the trip by another passenger who is at least 18 years of age.]
3. Separated or divorced custodial parent: Passengers under the age of 18 traveling between the United States
and any foreign country without his or her custodial parent (whether or not the minor is traveling with his or her
non-custodial parent) will need to provide Flight Options with a signed and notarized copy of the Consent Form
executed by the custodial parent and a notarized copy of the applicable child custody documents.
4. Separated or divorced parents with joint custody: Passengers under the age of 18 traveling between the United
States and any foreign country with only one of the custodial parents will need to provide Flight Options with a
signed and notarized copy of the Consent Form executed by the parent who is not traveling with the minor child
and a notarized copy of the applicable child custody documents.
5. Deceased Parent: Passengers under the age of 18 traveling with only one parent and the other parent is
deceased must provide Flight Options a notarized copy of the deceased parent's death certificate.
6. Legal Guardian: Passengers under the age of 18 traveling with both of his or her legal guardians, and not with
his or her natural parents, must provide Flight Options with a notarized copy of the applicable documentation
(e.g., guardianship decree) establishing that the individuals have legal guardianship of the minor. Minors
accompanied on an international trip by only one legal guardian must provide Flight Options a notarized copy of
the Consent Form signed by the minor's other legal guardian, if any, who is not traveling with the minor and a
notarized copy of the applicable documentation establishing that the legal guardian traveling with the minor and
the legal guardian signing the Consent Form have legal guardianship of the minor. If a minor is traveling with
only one individual who has sole legal guardianship, Flight Options must be provided with a notarized copy of the
applicable documentation establishing that the individual has legal guardianship of the minor.
If the Consent Form is not properly completed and any other required documentation is not provided, the
minor child will not be allowed to travel internationally. This requirement is strictly enforced and failure to
provide the signed and notarized Consent Form and other required documentation will delay the start of your
flight.
Individual foreign countries may have additional documentary requirements to establish the relationship of the adult to the minor and
permission for the minor to enter the foreign country. Parents and guardians are advised to contact the appropriate foreign government
agency to ensure that their child has the appropriate documentation prior to departure.
