Please attach the following documentation:
o certificate of Good standing (dated within 1 year)
o LLC Operating Agreement (copy of executed original, as amended, if
applicable)
o Addendum to operating Agreement listing the LLC owners (must include
ownership percentages)
o writing signed by LLc's Manager/Managing Member authorizing signors if they
are not specifically named in the operating Agreement
For signers on accounts: name, street address, Date of birth, country of
citizenship, social security #, photo id( if Darren Indyke we don't need this
information as it is on file).
