I believe Jeffrey must surrender his current passport to send in to request the 2nd passport. We will need to fill out a DS-82 form (which is normally used to apply for a passport renewal), a passport photo (which I have) and a letter of explanation. Polina, Jeffrey had a 2nd passport once before and I see a file in Darren's File Cabinet #2 in the JE misc file (drawer 3). You may find a previous letter that Darren has written for this and Darren could simply review the letter, make changes and you could type up(?) The website looks to be quite helpful.
