SPECIMEN
A: Lung, left upper lobe, wedge biopsy.
B: Lung, left lower lobe, wedge biopsy.
GROSS DESCRIPTION
A:
Received is a specimen fixed in formalin with patient name and labeled as
"1 upper lobe". It consists of a wedge biopsy of lung measuring 5 x 3 x 0.8 cm with
stapled line measuring 6 cm in length. The pleural surface is glistening, tan and rubbery.
Representative sections submitted in five cassettes.
B:
Received is a specimen fixed in formalin with patient name and labeled as
"2 lower lobe". It consists of a yellow-tan wedge of lung measuring 4 x 3 x 1.5 cm with
staple and focal nodule. Representative sections submitted in five cassettes.

MICROSCOPY DESCRIPTION
Benign lung tissue with acute areas of pneumonia and dilated air spaces with inspissated
material. There are some prominent vessels. There is new fresh fibroblastic
proliferation, and other areas of honeycombing with old fibrosis predominantly
interstitial. There is some nodularity. Focally, the fibrosis may arise in subpleural
region. Bronchioles overall are within normal limits. No tumor or granuloma. Focal
giant cells seen in part A, possibly foreign body reaction.
DIAGNOSIS
A, B:
Portion of lung, wedge biopsy:
* Acute inflammation with organizing pneumonia, possibly infectious etiology;
recommend correlation with culture.
* Interstitial fibrosis with areas of honeycomb lung, nonspecific vs. UIP.
