Who Must Attend:
Both the Petitioner and Beneficial), must attend this
appointment. Your attorney or authorized
representative may attend as well.
Required_Evidence To Be Submitted In-Person At Your
Appointment'
1) Photo identification for the Petitioner. Examples of
proper photo identification are U.S. passports,
driver's licenses, or state-issued identification cards. Store-
bought identification cards are not
considered proper identification. Failure to bring proper photo
identification may result in the denial
of the petition.
2) Proof of Petitioner's U.S. Citizenship, such as U.S. birth
certificate or naturalization certificate.
3) Beneficiary's passports (current and expired) and t-94 card.
4) Marriage Certificate and proof of termination (such as divorc
e decree or death certificate) of all prior
marriages or Petitioner and Beneficiary.
5) Birth certificates for children, current bank letters, bank
statements, lease agreements, rent receipts,
mortgage agreements. health insurance policies, life insura
nce policies, utility bills, tax returns, and
photos.
6) Currently-dated employment letters from both Petitioner's
and Beneficiary's employers.
Employment letters should he on company letterhead. The letter
must state the dale employment
began, salary, marital status, dependents claimed, and whom to
notify in case of emergency. The
letter must he signed by an official of the firm or company. The
latest two pay stubs or statements
must also be submitted.
7) Federal, state and local income tax returns for the most recent
tax year. Returns should he signed and
dated and proof of filing is required. Official IRS transcripts can
be obtained by calling 1-800-829-
1040. Attach W-2's for both Petitioner and Beneficiary.
8) Two passport-style photographs fur Petitioner and Beneficiary
(CIS-approved style).
9) OTHER:
NOTE: You must bring the originals and photocopies of all docum
ents submitted during the
appointment, including photocopies of your passport and identi
fication cards. Pursuant to 8 CFR
103.2(b)(1), the Service will retain for the record any origin
al document submitted as evidence unless a
photocopy is made. Please assemble your documents for submi
ssion in a neat and organized manner.
