Once this is received from both, I can grant eligibility. Also, to avoid needing these forms in the future, if the date
of crime is more than a year prior to the claim being submitted, we will need the section filled out on the
application. I attached a screen shot and highlighted where you can put in your reason. It can be didn't disclose,
didn't know about OVS, crime just discovered, etc. If you put the short explanation on the application, we won't
need any of the extra documents. Same thing if the crime was reported more than a week after it occurred.

In regards to Epstein there are some that stare they are victims that never spoke to us / and identified as victims. I can
look up the name to see if they are or not.

Another off topic question for you- I have a claim from an Epstein victim that was not put in by your office. I think
she put it in herself. Should I refer her to you or can I see if you guys already have her listed somewhere? I am
trying to get some sort of Criminal Justice report.

I am about to submit another claim for One Taste and the victim has the question about how therapist are validated. Does
she need to have the therapist write something or I told her I believe OVS sends a standard form to the therapist — is that
correct?

Hi
Once a claimant is eligible for counseling, they can send in any bills or receipts they may have. The therapist
can request reimbursement as well, rather than go through the claimant. The provider can send an email to
with the claim number and attach the bills/explanation of benefits, etc. Our Med unit
will reach out to the provider for the needed Mental Health Treatment form. Please let me know if you need
anything else!
