'was very apprehensive about having the immunisation Covid-19 due to her extensive me ica history of Acute allergy reactions to several medications and immunisations and having suffered the affects several times as a result, including suffering from 2 events of Anaphylaxis, one event being from the Influenza Immunisation 2014 , even following the expert opinion from Dr Adriana Le, Immunologist / Allergist, appointment in the Clare Practice. Royal Adelaide Hospital Specialist Vaccination Clinic Dated : 8th February 2020 Covid-19 Immunisation : Pfizer - As recommended by Dr Adriana Le, Immunologist / Allergist Prior to immunisation being administered : Allergist of the Royal Adelaide Hospital S ecialist Vaccination Clinic / patient was spoken to by the Allergist of the Specialist Vaccination Clinic, w o proceellilassess the patients Medical History of previous Acute allergy reactions to several medications, including the Influenza Immunisation in 2014 of which resulted EFTA00263319 RAH Allergist proceeded to assess the patient, reaction to the medication Olanzapine which she was informed resulted in an a ergy reaction of swollen lips, swollen tongue and respiratory distress 20 minutes followin administration of the medication RAH Allergist stated to the patient, following being informed of the allergy reaction to Olanzapine "that's cause for concern" (Red Flag not to proceeded with the immunisation) RAH Allergist then informed the patient, following the "Cause for concern" statement to proceed with the Covid-19 immunisatiorRi patient trusted the Allergist Professional assessment and proceeded to follow her professional medical advice The patient, was then administered the Covid-19 Immunisation following the assessment y e ergist and was kept in observation for an extended time for a 45 minute period as a precaution because of her Extensive Medical History of developing an Allergy reaction to the vaccine . The immunisation was administered at 10.35am - K Jones on the 8th February 2022 Batch # FL7649 The patient, did not suffer from any initial reaction to the immunisation, except for the usual in orme symptoms of the immunisation of flu like symptoms for 2 days 8 day delayed Immune response to the Covid-19 Immunisation on the 8th February 2022 On the advice of the 2 allergists involved and RAH , Adelaide Specialist Vaccination Clinic Eight days following administration of the Covid-19 Immunisation on the advice of the two allergist and the RAH Specialist Vaccination Clinic, and without the testing being conducted accordi ceived by Dr Adrian Le from Dr Daehn, Kadina Medical association the patient, began to show symptoms of a progressing Allergy reaction to the Immunisation. Symptoms included feeling unwell, swelling and congestion in the nasal, a rash that resembled a hive like rash, which was intensely itchy that covered the facial regions, arms, chest and legs, slight swelling in the facial regions. All symptoms that had been recorded in the Covid-19 Immunisation pamphlet of an Allergy reaction to the vaccine The symptoms of the progressing allergy reaction progressed severely over a five day period, resulting in severe symptoms of the hive like rash progressing to appear as severe skin lesions on her facial , neck, chest, arms, stomach and leg regions, severe swelling of the facial region, including nasal area, eye area, cheeks, lips, neck, a severe feeling of being unwell, severely swollen nasal passages (could not breath through nasal passages), congestion within nasal passages, a bronchitis type cough, severe tightness in the chest (respiratory system), shooting pressure type headaches in all areas of the head, severe burning and itching of the skin (couldn't stand to be touched on the skin without pain, which included bed sheets) , Respiratory distress, Insomnia, restlessness, irritability, dizziness that came and went, nausea that came and went Dr John Birss - Wallaro pital Emergency Ward - 23rd February 2022 The patient, presented herself at the Wallaroo Emergency Ward on 23rd February 2022 in Respiratory distress (rapid breathing) and displaying the severe acute symptoms of an Allergy Reaction, including Respiratory distress of rapid breathing to the Covid-19 Immunisation that was administered by the RAH Specialist Vaccination Clinic, under the professi • o allergist, Dr Adrian Le, and the RAH Allergist who assessed the patient, pi prior to the vaccination and advised her to proceed with the vaccination without anylorm of medical testing for an allergy reaction and consideration for the referral from Dr Daehn, Kadina Medical Association and the provided Extensive Medical History of the patient of Acute Allergy reactions to several medications and immunisations that had resulted in Anaphylaxis previously and the "Cause for concern" for the medication Olanzapine. EFTA00263320 The patient, was diagnosed as Acute Allergic Oedema to the Covid-19 Immunisation administered on the 8th February 2022 at the RAH Specialist Vaccination Clinic at 10.33am by K Jones • nal medical advice of the two allergist consulted and assessed the patient, because of her extensive Medical History of Acute allergy reactions to several medications and including immunisations, which included the Influenza Immunisation in 2014 and ADT in 2020, resulting in Anaphylaxis and Allergy asthma The patient,.. was administered a prescription for the medication Prednisolone 25mg( Cortisone an a me ical plan, which involved : 2x 25mg tablets daily for 3 days lx 25mg tablets daily for 3 days 112 25mg tablet for 4 days - then stop medication The patient,... was informed to follow the medication plan for a second course if symptoms persis e Symptoms persisted f John Birss the patient, • urseof the medication plan and under the advice of Dr proceeded to administer a second course of the medication plan persisted following the second course of the medication plan. The patient, proceeded to consult with a GP, Dr Kwan Chia, Kadina Medical Association at 9.45am , arch 2022 and was instructed to c • until symptoms are resolved. The patient, is symptoms with Phenergan 25mg proceeded to administer the medical plan advised by the GP of Phenergan 25mg until symptoms resolve. Ongoing symptoms of the Acute Allergy Oedema following the Covid-19 Immunisation did not resolve following the applied medical plan of Phenergan 25mg 5th March 2022 - Following Medical treatments of Acute Allergy Oedema to the Covid-19 Immunisation - Conducted without the correct Medical procedure of testing by two allergists resultin • the delayed Immune system response of Acute Allergy Oedema The patient, 8th March 2022 is still suffering from symptoms of allergy reaction to the administered Covid-19 immunisation and the immune response of Acute Allergy Oedema, which include, sneezing, congestion in the nasal passages, triggered allergy asthma and reoccurring hive like rash on chest region and facial region (flare ups) 28th March 2022 - Dr Daehn, Kadina Medical association Appointment : 11am The patient,EME. was advised by Dr Daehn, Kadina Medical Association that further investigation , including testing was essential to her medical safety to find the reactive allergen involving medications and immunisations and proceeded with a referral to an allergist, Dr Frank Kette, entre The patient, cancelled the appointment involving the referral to the allergist, Dr Frank Kette, Adelaide Allergy Centre because of her suffering from Extreme Anxiety of having medical procedures conducted, as result of the Acute Allergy Oedema she suffered as a result of the administered Covid-19 Immunisation and the medical negligence and endangerment to her life of the two allergists involved, who did not conduct testing for an allergy reaction according to her Extensive medical history of allergy reactions to several medications, including immunisations prior to their medical professional opinion to proceed with the Covid-19 immunisation file in the delayed immune response of Acute Allergy Oedema and the patients, suffering and endangerment to her life (Rapid breathing, Extreme Respiratory distress) Psychological effects of the Medical neglect and endangerment -
