Intake on 7/8/19. Was found unresponsive on 8/10/19 with ligature marks around neck and posterior mottling. Noted to previously have been on Toprol XL — Not ordered; however, all blood pressure and pulse readings in flow sheets within normal limits. Had a cell mate, and interacted with him. Noted that he did not appear to be in distress and SHU staff did not report any unusual behaviors. Noted to have an incident with string/rope around neck causing damage. Denies self-harm or knowledge of the incident. Was drinking water and then woke up snorting.
