Intake on 7/8/19. Was found unresponsive on 8/10/19 with ligature marks around neck and posterior mottling. Medication history: Psychotropic medications: None Non-psychotropic medications: Sliding Scale Insulin (expired 8/7/19), Omega 3 (Vascepa), Docusate, Milk of Magnesia OTC medications: Vitamin C, Halls, Hydrocortisone, Tylenol, multivitamin Compliance/adherence: Adherent per self-carry refill history Drug-Drug interactions: None Drug-Disease interactions: None Pertinent objective data: 7/30/19: Seen by physician for numbness in right arm, sleep apnea. Noted to previously have been on Toprol XL — Not ordered; however, all blood pressure and pulse readings in flow sheets within normal limits.
