Trusts and Nominees Please provide ALL of the following: (a) name of the settlor(s); (b) a copy of the original dated executed trust deed; (c) evidence of the identity of the current trustees and a certified copy of the deed evidencing their appointment; (d) in the case of a corporate trustee/nominee, the same requirements apply as those for listed or unlisted companies, as the case may be, set out above; (e) in the case of regulated professional trustees/nominees, please provide evidence of regulation and confirmation in writing that such professional trustees/nominees are aware of the true identity of the beneficiaries/settlors and that there are no anonymous principals; (f) in the case of other trustees or nominees, please provide written confirmation and documentary evidence of the identities of all the settlors and named beneficiaries with vested interests of 25% or more in the trust; and (g) include unexpired government id (with clear photograph), including residential address, for all beneficiaries, including trustees, if not equivalently regulated, and the senior (if available) and ensure that an authorized signature list accompanies any trust document.
