Dulce saw orthopedic surgeons in the DR and here. The one here does the surgeries at Baptist Hospital in

Alternatives to Surgery
Dulce has been on a low sodium diet and has lost more than 20 pounds per the doctor's instructions. She has
been on glucosamine for about three weeks per your suggestion. She is also on a regimen of prescription drugs
for pain management. She says she cannot yet tell a difference.
Clarity around Dulce's Surgical Needs
1. Both knees carry inflammation because the cartilage has deteriorated, but one is clearly more inflamed and
painful for Dulce than the other.
2. The tendon in the back of Dulce's leg was nearly severed during an accident with a glass table while here at
work. As I understand it, this occurred right around the time of the hurricane, and therefore, it was not properly
repaired. It does not cause her so much pain, but she cannot lift her toes which has changed her gate.
Surgical Options - Please Choose One
Option A: Do nothing. Release Dulce. She is on her own.
Option B: Do the worse knee. She will be on her own for the second one.
Option C: Do the worse knee. See how it goes. We'll help her do the other knee at a later time if it works.
Option D: Do both knees at the same time. She will be on her own for the tendon repair.
Option E: Do three separate surgeries: 1.) the worse knee, 2.) the tendon repair, 3.) the other knee.

Option F: Do the tendon. She is on her own for the knees.
Plans for Transitioning Housekeeping Staff
1. I understand we are running a business and that you want to transition the entire housekeeping staff
2. The hesitation you see in me is the consequence of added training and supervision this will require from me
since everyone will be inexperienced with your space and preferences. I want to perform well for you in
making sure all your needs and request are met.
3. Hiring a cleaning company to get in, do their thing, and get out makes the most sense to me. Because we'll
be paying them for what they accomplish instead of for the hours they're here, this will require less supervisory
time from me while ensuring the job gets done in a timely manner.
4. Even with her knee troubles, Dulce is by far the most valuable and works hardest of the four ladies. She
carries years of institutional wisdom from which I need to glean. She knows everything from what kind of t-
shirts you prefer to the exact steps for hurricane preparation. If her knees are fixed, she is otherwise strong and
in good health. Instead of four full-time persons, she could be a solo lead in keeping a hawk's eye on the
cleaning company and picking up whatever needs occur when they are not here, freeing me to focus on other
things. I can designate Okeam (Kilo - the young man who works very hard as my right hand person who was
helping us in the tunnel) to help her as needed. This will significantly reduce the staffing needs in
housekeeping while ensuring a tidy and well-stocked environment.
