Unfortunately, we need those on every claim. On the application, there is a section on the front page to write a little blurb
as to why the victim is applying after a year and why they reported to PD after a week. While I totally understand both
answers to the questions in these cases, if the sections on the claim are not completed, the victim must fill both of those
out. This will be the same for all of the victims if that section is not filled out. All that needs to be put in those spots on the
app are something as simple as "victim didn't come forward/ victim was afraid to report." Unfortunately these forms are
legal forms and we cannot approve a claim without either the app part filled out or the late OVS filing and late PD
reporting form. If you have any new applications coming in, just make sure the section is filled out and then no forms
would be needed. Let me know if you have any questions.

Thank! I just spoke to their attorney Brittany and she said that the letter was requesting an form
stating why the claim was submitted late. Do you need that or knowing this is the Epstein case resolves that
question?

Hi and Intake- I am following up on two applications that you have asked for the FBI case 1i; I can't find
the other email but I believe it is this one and I wanted to
touch base to get these applications opened. The case agent interviewed them both and confirmed they are
both victims of Jeffrey Epstein 31E-NY-3027571. Both date of crime occurred between 2003 and 2004.
